Evaluation Reservation Form -

Since this is your dogs first visit, we require a 2 hour evaluation to make sure that your dog will do well in our daycare and hotel setting. Evaluations may be done any day from 9am-2pm. We say that the evaluation is 2 hours, but you can leave the dog longer, we just ask that you are available the first 2 hours. You do not need to stay for the evaluation, our staff will keep a close eye on your dog. Please bring a current copy of your dogs vet records showing proof of Rabies, Bordetella, and DHPP. If the Bordetella vaccine has never been given or has lapsed in validity, it needs to be administered at least 5 days prior to your pet’s stay. We only accept social dogs that do well with other dogs in our free play setting. If you would like to save time, you can print out our Click here and bring it with you on your first visit.
